The ornate certificate consists of red and black printed imagery including the Roman goddess Minerva or Victoria, two soldiers, the Royal coat-of-arms of the United Kingdom and various foliate and architectural motifs. The text reads: '9/1053 Trooper Charles John Hayward / New Zealand Expeditionary Force / Served with honour and was disabled in the Great War / Honourably discharged on 13th November 1919 / [signed] George R J'.
